A free Continuing Medical Education (CME) Conference for primary care physicians and providers, practice administrators and quality professionals. This conference is designed to update community primary care practices on the evolving guidelines for hypertension management and treatment. The intent is to inform the attendees on innovations in treating special patient populations; these innovations include new drugs, technology, and community health workers. This conference features local and national experts and is jointly sponsored by the Icahn School of Medicine at Mount Sinai and Healthfirst.

Program Objectives

At the conclusion of this activity, participants will be able to:

Objective 1: Review the latest evidence-based guidelines for hypertension management

Objective 2: Identify and discuss barriers faced by special populations that are disproportionately affected by hypertension

Objective 3: Integrate approaches to improve hypertension management of special populations in community-based practices

Objective 4: Promote the use of pragmatic strategies to achieve patient engagement and hypertension control
